"Where does the ASCII format of tab come from, what generates it?"
Usually as a file from a software tab program, like Tabledit. I can export tabs to several different formats, i.e., ASCII, ABC &/or TXT, or print PDFs. I imagine you could also format such a file in a word-processor program, too. You would need to make sure the format was a fixed font.
" Second question on ASCII, is the spacing always in quarter or eigth notes?"
That is probably because most people tend to use 2/4 or 4/4 time in tabs to keep it from being too busy. If you were to write the tab in 16th notes, I'm sure the exported file would show 16th notes, but an entire tab of 16th notes is really hard to follow.
